You know, the same thing happened to me! For more information, read 'Does he still like me?' (18th December).
Anyways I decided to do something about it because it was just killing me inside...and I didn't want to let the opportunity slip away. We were texting and I wanted to know if he still likes me, and if he still does I decided to let him know how I feel. Anyways he said, 'Nah, you know that I would never want to leave you!' That was just so perfect what he said and clearly it showed that he still had feelings for me, so I asked, 'Do you want to give things a go then?' And yeah :) We're going out now.
So what you should do, is find out for sure that he still likes you (I reckon he does!) but always make sure. You don't want to tell him only to realise that he's got himself a new girlfriend or likes some other girl...So drop little hints, ask him questions...anything to find out if he still feels the same.
And once you do find out that he does still like you, just tell him straight! I am sure he will be glad to hear it from you :) Good luck girl! Let me know how it goes.
